c. Power of attorney duly authenticated d. Present the respective payment receipt to SENASA in accordance with Article 3, numeral 6, of the "Regulations on fees for services provided by the General Directorate of the National Service of Health and Food Safety (SENASA, for its acronym in Spanish) (Agreement No 005-2019)", which sets fee for the Distribuitor of Fertilizers and Pesticides Registry, at L 4,725.00 (four thousand seven hundred and twenty five Lempiras). e. Name and legal address of the Distributor of the Company. f. Name of responsible technical adviser. g. Owner of the Pesticide Registry to be repackaged or approved by the Department of Control and Use of Pesticides. h. Company´s Articles of Incorporation and appointment of representatives. i. Record of registration in the corresponding official units. j. Location, full address of the reprocessing plant, warehouses, or storehouses. k. Certification of training of sales personnel. l. Description of pesticide storage facilities, safety conditions. m. Contingency plan. * For the corresponding payment, the establishment requests a payment authorization slip to the Department of Control and Use of Pesticides. This authorization must be submitted at the Regional International Organization for Plant Protection and Animal Health (OIRSA) office which delivers a receipt, which must be submitted to the BANADESA Bank counter in SENASA. VALIDITY: The Register of Establishment shall be valid for 2 years, and for its renewal it shall comply with the registration requirements set out in this section, as appropriate. Any request for renewal must be made 30 days before its expiration. 6.10.5 APPLICATION FOR REGISTRATION OF IMPORTER OF FERTILIZERS AND PESTICIDES The application is regulated under the Phytozoosanitary Law, Decree No. 157 94, art: 14 and 15 - Regulations on use and control of pesticides, art: 6, 7, 8, 9, 51 subsection “d”, 194, 195, 196 y 197. Current Regulations, Agreement No. 005-2019 on SENASA service fees art: 3 numeral "3". REQUIREMENTS: a. Application for registration addressed to the Director General of the National Service of Health and Food Safety, presented by the Legal proxy of the Establishment which conferred his power. b. Complete the form and attach to the application.
